Mother To Donate Uterus To Daughter For Experimental Operation

A British woman born without reproductive organs is hoping to conceive by undergoing an operation for a uterus transplant -- supplied by her own mother.

Sarah Otterman, a 25-year-old biology teacher who lives in Stockholm, was diagnosed with Mayer Rokitansky Kuster Hauser syndrome at birth and cannot have children, though she has considered adoption.

But now she's a potential candidate for a womb transplant trial in Goteburg, Sweden, which will test the success of the experimental procedure in ten pairs of donors and recipients. If the organ is not rejected after one year, in-vitro fertilization (IVF) drug therapy will be administered to boost the patients' chances of conceiving after organ transplantation.

After much discussion, Sarah's 56-year-old mom, Eva Otterman, a managing director at a Nottingham lighting company, volunteered to be her daughter's donor. In fact, the majority of the donor pairs are mothers and daughters, something that didn't surprise Dr. Mats Brännström, head of the transplant team. He told the Toronto Star:

It is the natural fit. There may be an advantage with the mother because the daughter is similar in tissue type so you have less of rejection. The mother would also be more determined to help their daughter in this way than other people.

Despite the odd circumstances, it's a true example of deep motherly love. Brännström and his team have already done trials with 25 female baboons -- though without the IVF therapy -- and estimates a 60 to 70 percent chance of success with humans based on preliminary data. If the first five human womb transplants to be done in 2012 are successful, this procedure could eventually become available worldwide to thousands of women who cannot naturally conceive. All of this leads us to ask, would you be willing to donate or receive a womb transplant from a relative?

Scientists are growing human hearts in laboratories, offering hope for millions of cardiac patients.

American researchers believe the artificial organs could start beating within weeks.

The experiment is a major step towards the first ‘grow-your-own’ heart, and could pave the way for livers, lungs or kidneys to be made to order.

The organs were created by removing muscle cells from donor organs to leave behind tough hearts of connective tissue.

Researchers then injected stem cells which multiplied and grew around the structure, eventually turning into healthy heart cells.

Dr Doris Taylor, an expert in regenerative medicine at the University of Minnesota in Minneapolis, said: ‘The hearts are growing, and we hope they will show signs of beating within the next weeks.

‘There are many hurdles to overcome to generate a fully functioning heart, but my prediction is that it may one day be possible to grow entire organs for transplant.’

Patients given normal heart transplants must take drugs to suppress their immune systems for the rest of their lives.

This can increase the risk of high blood pressure, kidney failure and diabetes.

If new hearts could be made using a patient’s own stem cells, it is less likely they would be rejected.

The lab-grown organs have been created using these types of cells – the body’s immature ‘master cells’ which have the ability to turn into different types of tissue. The experiment follows a string of successes for researchers trying to create spare body parts for transplants.

In 2007, British doctors grew a human heart valve using stem cells taken from a patient’s bone marrow.

HOW TO GROW YOUR OWN HEART

  • The donor heart is removed from the body; pig hearts may also be suitable.
  • Detergents are then used to strip the cells from the heart leaving behind the protein skeleton or 'ghost heart'.
  • Stem cells grown from cells taken from a patient are then added to the ghost heart.
  • The stem cells then multiply and generate new heart cells. now all that is left is the hope that these will start beating.

A year later, scientists grew a beating animal heart for the first time.

Dr Taylor’s team have already created beating rat and pig hearts. Although they were too weak to be used in animals, the work was an important step towards tailor-made organs.

In their latest study, reported at the American College of Cardiology’s annual conference in New Orleans, researchers created new organs using human hearts taken from dead bodies.

The scientists stripped the cells from the dead hearts with a powerful detergent, leaving ‘ghost heart’ scaffolds made from the protein collagen.

The ghost hearts were then injected with millions of stem cells, which had been extracted from patients and supplied with nutrients.

The stem cells ‘recognised’ the collagen heart structure and began to turn into heart muscle cells.

The hearts have yet to start beating – but if they do, they could be strong enough to pump blood.

However, the race to create a working heart faces many obstacles.

One of the biggest is getting enough oxygen to the organ through a complex network of blood vessels. Scientists also need to ensure the heart cells beat in time.

Dr Taylor told the Sunday Times: ‘We are a long way off creating a heart for transplant, but we think we’ve opened a door to building any organ for human transplant.’

U.K. Woman with Four Transplanted Organs Gets Her M.D.

With Donor Heart, Lungs, Kidney, and Liver, She'll Now Be Doing Rounds

The first woman in the United Kingdom to have all her major organs transplanted has now signed on to a lifetime of hospital visits -- but this time as the doctor.

Allison John, 32, made medical history in 2006 after she received her fourth organ transplant -- a kidney from her father, 61-year-old David John, to add to her previous heart, lung and liver transplants.

A life plagued by illness and frequent hospital visits has not deterred John from her dream of becoming a doctor, however. After 14 years of interrupted study, she finally received her medical degree from Cardiff University last month, according to the U.K. press.

John will begin rounds as a junior doctor at Neville Hall Hospital in Abergavenny, England.

Considering that her combined heart-lung transplant alone has only a 40 percent survival rate in the first five years, according to the United Network for Organ Sharing, John is extremely fortunate to have survived three separate transplant operations.

"At the moment, I feel I'm in the best shape physically that I've ever been and I'm so excited about the future," John told the U.K. press.

Doctor-to-Be Survives Full Major Organ Transplant

Why would one person need so many organ transplants?

Just weeks after her birth, John was diagnosed with cystic fibrosis, an incurable genetic condition that results in the formation of thick mucus that plugs up the tubes leading to the lungs and liver -- though other organs also can be affected by it.

"Cystic fibrosis can result in lung and liver damage, as the thick mucus causes inflammation and infection that destroys [organ] tissue," said Dr. David Cronin, associate professor of transplant surgery at the Medical College of Wisconsin.

According to the U.K. press, John's liver was transplanted first, at age 15, but just a year later, doctors told her she would also need a lung transplant and she received a combined heart/lung transplant less than a year later.

The Miracle of Multiple Transplants

Given she already had undergone a liver transplant, it might sound strange that doctors would give John a lung and heart transplant once her lungs failed, but there are advantages to adding the heart into the mix, Cronin said.

"At the time that transplant was done, doing heart and lung transplants together was much safer and easier than doing an individual lung transplant," he said. "When you do both, you take the heart and lung out as one unit from the donor and put it into the recipient. Basically, you're doing a heart transplant and the lungs come along for the ride."

Also, the liver failure experienced by those with cystic fibrosis can put extra stress on the heart, so transplanting the heart, as well can be beneficial to survival, said Dr. Fredric Gordon, medical director of liver transplantation at the Lahey Clinic Medical Center in Massachusetts.

Would each successive transplant surgery be riskier than that last?

Not necessarily, Cronin said. While risk of infection would be there with each operation, the past transplants, once established, would not necessarily increase the risk of the next ones.

She also already would be on immunosuppressive drugs, so she wouldn't have to start those or increase the dosage for the subsequent surgeries, Gordon said.

Ironically, but not surprisingly, John's final organ transplant actually was a product of her first three. Organ recipients must be on a lifetime regimen of drugs to suppress their immune system in order to stave off the body's rejection of the new organ. The drugs however, have side effects that can slowly weaken the kidney.

"John had what 30 percent of liver transplant patients and 40 to 50 percent of heart transplant patients develop in the five years after transplant: kidney failure as a side effect of the immunosuppressive drugs," said Cronin.

Given that John will have to remain on immunosuppressant drugs for the rest of her life, her donor kidney may suffer the same fate as her original one because of the drugs, Gordon said.

It is a problem that transplant surgeons only have been able to detect in more recent years, as patients survive long enough to experience that particular side effect five, 10, 20 years down the road, Cronin said.

"[John] has done miraculous[ly]," Cronin said. "It's more than 10 years and it looks like she's full steam ahead."

Doctors in Spain claim to have performed world's first full face transplant

A team of 30 Spanish doctors claim to have successfully performed the world’s first full face transplant.

 

Doctors in Barcelona who performed the latest surgery said that it was the first full face transplant to be carried out anywhere in the world.
As well as skin tissue, the patient received new cheekbones, facial muscles, teeth, palate, skin, nose, lips and jaw.

The gruelling operation took 24 hours in total to perform on March 20.

Before the surgery, the man, a farmer in his thirties, had been unable to swallow, speak or breathe properly after accidentially shooting himself in the face in 2005.

The man, who has not been named, is expected to remain in hospital for at least two months.

A statement released by Barcelona's Vall d'Hebron University Hospital, where the operation was carried out, said: "(The patient) had been operated on nine times without satisfactory success, therefore he was considered for full face transplant.

"This is the first full face transplant performed worldwide, as the 10 operations performed previously had been only partial."

The world’s first face transplant was carried out on a French woman, Isabelle Dinoire, in 2005.
She had been ravaged by her dog and received new facial tissue as well as a new nose and mouth.
Since then there have been a number of such operations, including in China and America.

In 2008 an American woman, Connie Culp, received what was at that point the most extensive face transplant ever performed.

As well as skin she received bone, muscle and blood vessel and nerve transplants.

Prof Peter Butler, head of the UK's Facial Transplantation Research Team, has been ready to perform a full face transplant for several months.

His team, based at the Royal Free Hospital in North London, has ethical permission to perform four face transplants.

He praised the work of the Spanish team, led by Dr Joan Pere Barret.

He said: "We congratulate Dr Barret and his transplantation team in Spain on what may well be the most complex facial transplantation operation carried out so far worldwide.

"Secondly I would like to wish the patient well for the future. 

"We must also remember the family of the donor who, we understand, has helped not only the facial transplantation patient, but others, with various forms of organ donation.

"To help others, not only to live but to have a good life, is a supreme act of human generosity.”

Donor says he got thousands for his kidney

Editor's note: Since the FBI arrested a Brooklyn businessman in late July on federal charges of organ trafficking, CNN has been conducting a worldwide investigation into the sale of kidneys using willing donors and willing recipients from China to Israel to the United States.

TEL AVIV, Israel (CNN) -- Four years ago, a young, cash-starved Israeli answered an ad in a newspaper for a kidney donor.

"I decided I wanted to make a positive change in my life and do something different," Nick Rosen told CNN. "So I saw an ad in the paper and it said, 'Kidney Donor Wanted.' And called the ad in the paper, and they asked me my blood type."

Ultimately, Rosen flew to New York and underwent surgery at Mount Sinai Medical Center to remove one of his two healthy kidneys.

"Let's say I donated a kidney and received compensation," he said.

Rosen's story is one of several that have come to light in recent weeks as part of a worldwide CNN investigation into what appears to be a widespread black market in human organs currently under scrutiny by authorities in the United States and Israel.

Rosen says he was paid $20,000 for his kidney -- something he admits he lied about in interviews with the hospital's transplant team.

What Rosen did -- and what the man who received the kidney did -- violated a 1994 U.S. federal law that forbids the selling or purchase of live organs for cash. He not only got his money, but made an 11-minute documentary film he called "Kidney Beans" to show how easy it was to sell an organ. A portion of the documentary shows him lying on a bed, covered in cash he says he was paid.

In a written statement, Mount Sinai told CNN: "The pre-transplant evaluation may not detect premeditated and skillful attempts to subvert and defraud the evaluation process."

"Mount Sinai's transplant screening process is rigorous and comprehensive, and assesses each donor's motivation," the hospital said.

A hospital medical source put it more bluntly: "We were duped."

According to kidney transplant doctors, the process of pre-screening, blood-type matching and other related medical issues normally takes two months before any surgery. During that time, both recipient and doctor have to make several visits to a team of doctors, social workers and perhaps even ethicists before a final decision is made.

Hospitals often ask donors to sign documents which ask whether they have received any compensation for donating a kidney or other organ. But no documentation is required to prove a family connection.

The chief of nephrology at Mount Sinai later said hospitals and doctors are primarily concerned with medicine. "We're not detectives. We're not the FBI," Dr. Barbara Murphy said. "People can, on occasion, deceive us."

But what Rosen did was not unique, according to the World Health Organization, which says 10 percent of kidney transplants worldwide are believed to be illicit.

Dr. Eli A. Friedman, a leading kidney specialist, teacher and researcher at Downstate Medical Center in Brooklyn, said the United States leads the world in kidney transplants. About 16,000 of them are performed every year, he said.

"That would mean that somewhere between 1,500 and 2,000 transplants in the United States might fall in the categorization of being illegal," Friedman said.

He added, "I have had several experiences with patients with data that says they got a kidney from their brother, their sister or from their parent when they don't have a brother or a sister, " Dr. Friedman told CNN. " The transplant was performed under false circumstances."

And Friedman said he's been offered bribes of $5,000 to $10,000 by kidney brokers.

"Of course I was very happy to see them leave the office very rapidly," he said.

The extent of the underground network came to the surface in late July, when FBI agents arrested a Brooklyn businessman on charges of organ trafficking. A federal complaint against Itzhak-Levy Rosenbaum said he had offered to provide a new kidney for a relative of an undercover FBI agent for about $160,000.

According to the complaint, Rosenbaum told the agent he could buy the kidney for about $5,000 and gave instructions on exact procedures and methods to avoid detection. The complaint quoted Rosenbaum as bragging: "So far, I've never had a failure."

Ronald Kleinberg, the attorney for Rosenbaum, told CNN he could not comment on the FBI complaint "because I have not had enough time to assess the information." He said CNN's "assertions are incorrect," and that law enforcement's account of Rosenbaum's network was inaccurate.

But law enforcement sources said Rosenbaum had been the centerpiece of a kidney-for-sale operation, which he called "United Lifeline," that operated extensively for nearly a decade.

The donors and patients in this network were linked by one common theme -- they were Jewish. Investigators say the donors usually came from Eastern Europe, were mostly poor and willing to sell their kidneys to U.S. and Israeli patients.

According to one expert on organ trafficking, the FBI had been alerted to Rosenbaum's activities years ago.

Nancy Scheper-Hughes, an anthropologist at the University of California at Berkeley and founder of a newsletter called "Organs Watch," said she had told the FBI about Rosenbaum and her suspicions about him seven years ago.

"I think they thought it was a very few bad apples," Scheper-Hughes said.

The FBI said would not officially comment on her assertions, but an FBI source later said, "We developed our own leads."

According to Scheper-Hughes, the same day she spoke to CNN, she had learned of another illegal transplant surgery taking place at Cedars-Sinai Hospital in Los Angeles. The donor, she said, was a young Korean national who had received more than $25,000 for one of his kidneys.

"This is a kid that does not speak much English, who is terrified and shaken," she said. "And he thought, 'Maybe I've made a mistake to do this, but $25,000 is a good amount of cash.'"

Scheper-Hughes said the cash had been handed over in two increments, with the second paid to a relative in a hospital bathroom.

In a statement, Cedars-Sinai spokeswoman Sally Stewart said living donors "must state they are not receiving payment for their kidney."

"If at any time during the evaluation process, the transplant team suspects the donor is inappropriately being paid for a kidney, the transplant is canceled," she said. But a hospital source later told CNN, "We do not give lie-detector tests to our patients."

According to Scheper-Hughes, who is in the final stages of writing a book on organ trafficking, much of the world's illicit traffic in kidneys can be traced to Israel.

"Israel is the top," she said. "It has tentacles reaching out worldwide."

Until March 2008, Israeli law allowed Israeli citizens to go abroad for live organ transplants from non-related donors. But there was no way for Israeli authorities to keep track of how many of those cases involved money changing hands, the country's Health Ministry said.

Israeli investigators are looking intensively at illegal organ trafficking under the new law, the ministry said. And prosecutors in the West Bank town of Nazareth sent nine Israelis to jail in 2007 after uncovering a black-market ring that was buying and selling organs.

Gilad Ehrlick, the assistant district attorney for Israel's Northern District, said he was shocked by the case. Secretly recorded conversations showed that Arab and Russian newspapers were targeting low-income Israelis and Palestinians with ads saying there would be payment in exchange for providing a kidney.

"The idea was the people were calling out of despair, out of urgent need who needed a quick way to make money," he said.

In double transplant, left hand works first

WASHINGTON (AP) — When patients had both hands transplanted, their brains re-established connections much more quickly with the left hand than the right, a team of researchers in France reports.

The sample was small, just two patients, but both had been right-handed before losing their hands, and both followed a pattern of reconnection with their brain that was quicker for the left hand.

The study, led by Angela Sirigu of the Center for Cognitive Neuroscience at the University of Lyon, France, is reported in Tuesday's edition of Proceedings of the National Academy of Science.

The research shows that even years after the loss of hands the brain can reorganize and rewire itself to recognize and connect to a replacement.

And it comes just days after French physicians, in a 30-hour operation, performed the world's first simultaneous partial-face and double-hand transplant. Paris' Public Hospital authority described the recipient as a 30-year-old burn victim who was injured in a 2004 accident.

Sirigu's team used magnetic imaging to study the brains of people who had lost both hands and to see how the motor region that controls movement responded after new hands were transplanted.

The first case involved LB, a 20-year-old man injured in 2000, who received the transplants in 2003 after having used artificial hand devices in the meantime.

He was checked periodically and the researchers found his brain had re-established nerve connections to control the left hand by 10 months, while it took 26 months to complete the rewiring needed for the right hand.

"Interestingly, despite that LB was right-handed, and that after his amputation he used his prosthetic device mostly with his right hand, hand preference shifted from right to left after he had the graft," the researchers reported.

The second patient studied, CD, was a 46-year-old man who lost both hands in 1996 and received a dual hand transplant in 2000. He was tested by the researchers in 2004, 51 months after the transplants. Strong connections in the brain were observed for the left hand, but not yet the right.

The researchers said more study is needed to determine why the brain reconnected more efficiently to the left hand in these patients. Possibilities include a basically better connection to the left hand, factors in the way that the brain reorganizes itself during the process of the loss of a hand and its later replacement, or perhaps some pre-existing difference in brain organization.

In general, experiments have shown that the right side of the brain controls the left side of the body and many researchers believe it also dominates in such areas as spatial abilities, face recognition, visual imagery and music. The left side of the brain controls the right side of the body and is thought to dominate in language, math and logic. However, many traits are shared by both sides, and if one side is damaged the other can take over many of its functions.

The research was supported by the U.S. National Science Foundation, the French National Center for Scientific Research, the International Brain Organization and other organizations in Brazil, France and Canada.

Kidneys grown in genetically modified mouse

SCIENTISTS have successfully used implanted cells to create kidneys inside a mouse whose parents were genetically engineered so their offspring would not be born with the organs.

The team, headed by Professor Hiromitsu Nakauchi of Tokyo University's Institute of Medical Science, extracted a fertilised egg obtained through the normal mating of the genetically modified parents.

The embryo was then implanted with induced pluripotent stem, or iPS, cells from a mouse with kidneys and then transplanted into the womb of a surrogate mother.

The baby mouse was born with kidneys and it is believed its bladder inflated and it produced urine as normal, MCT news agency reported

Implanted iPS cells are thought to have compensated for the kidneys the mouse should have been born without.

The team now plans to use the research to create internal organs of monkeys inside pigs.

"If we become able in the future to create human kidneys inside pigs, we'll be able to solve the problem of a lack of organs for transplant surgery," Prof Nakauchi told MCT.
